What is Interprofessional Collaborative Care (IPCC)?

When multiple healthcare workers from different professional backgrounds work together with patients, families and communities to deliver the highest quality of care. (World Healthcare Organization, 2010)

  • The patient IS the CENTRAL member of the team.
  • The physician, nurse and pharmacist meet with the patient and family AS A GROUP upon ADMISSION.
  • The physician, nurse, pharmacist and case manager will round daily AS A GROUP to meet with the patient and family.
  • The PLAN OF CARE is developed TOGETHER with the PATIENT

What is Relationship-Based Care (RBC)?

Relationship-

A patient-centered healthcare model that is built on the premise that positive human connections support the healing process. (Creative Health Care Management, 2012)

RBC is comprised of 3 CRUCIAL RELATIONSHIPS:

  • Care provider relationship with patients/families
  • Care provider relationship with self
  • Care provider relationship with colleagues

The newly developed Excela Health “ Patient Care and Experience Pathway” schematic identifies specific Care Strategies for use on Admission, During Hospitalization and during Transitions of Care.

Weaved throughout the patient’s hospitalized experience are the three core concepts of:

  • Relationship-Based Care
  • Collaborative Practice Innovations
  • Interprofessional Education and Practice

The GOAL of the pathway is high reliability, interprofessional collaborative teams who deliver safe, effective, timely, efficient and equitable care that is patient-and family-centered.
