Of all the relationships we develop in life, one of the most important is that between the patient and the physician.  Primary care providers (commonly referred to as PCPs) include a full array of providers to take care of you, including family medicine physicians, internal medicine physicians, advanced practice providers such as certified registered nurse practitioners (CRNPs), and physician assistants (PA-Cs).  Your primary care physician will get to know you – your medical history, family history and other health-related concerns to offer you the best care possible. Our focus is on keeping you healthy and partnering with you to manage your health. Together, we will build a lasting relationship centered around your health.

Our primary care providers follow the “Patient-Centered Medical Home” model. This means that they value care coordination and communication the way you, as the patient, would. Your well-being is at the center of all they do. They will direct your care toward specialists or other services, if needed, and help you manage your overall health with a focus on prevention and wellness.
