As one of the oldest, most experienced non-profit hospice services in Westmoreland County, Excela Hospice is committed to providing the highest level of palliative care to patients and families faced with a terminal illness.
Medicare Certified Program
Our hospice program is Medicare certified and provided under the approval and supervision of your family physician. Care is coordinated by our skilled nursing staff and may include symptom control, pain management and physical, emotional, psychosocial, spiritual and practical support for patients and their families.
Professional Hospice Team
Each patients plan of care is supervised by our medical director and delivered by our specially trained hospice care team. Our team works closely together and with patients and family members to provide meaningful, end-of-life care and support.
Team members include:
- Skilled Nurses Provide medical supervision and nursing care, patient education, family instruction and supportive services
- Social Workers Coordinate resources and provide support and counseling
- Home Health Aides Help with personal care
- Physical, Occupational and Speech Therapists Help improve the quality of everyday living with therapies when needed
- Bereavement Coordinator Discusses concerns and helps loved ones work through their grief
- Dietitian Provides nutritional support as needed
- Hospice Trained Volunteers Provide a support system for caregivers by visiting patients or helping you and family members with errands
- Clergy Offer spiritual and emotional guidance, as requested
