A choice shared by patients and their families
To receive hospice care, patients must choose to receive the service. Family members, your physician, a clergyman or other health care professionals can also refer patients to hospice.
Eligibility
To enter hospice, a certified prognosis of six months or less from a physician is required. Patients benefit the most when they are able to receive a full six months of hospice care.
While those with cancer make up a large portion of our patients, our hospice program is also appropriate for persons with other terminal illnesses, such as lung disease, end-stage heart disease, Alzheimer’s disease, multiple sclerosis, Parkinson’s disease and amyotrophic lateral sclerosis (ALS).
Treatment Process
Once a referral is made, an individualized plan of care and treatment is developed based on your physician’s orders and an overall assessment by our hospice care team. Patients and family play an integral role in this process.
The attending physician is routinely informed of the patient’s status, and the care plan is adjusted as needed. Patients and families are encouraged to ask questions and actively participate in their care and treatment, particularly when assistance or reassurance is needed.
